We are asking all New Clients to complete a Confidential Extended History Form, and bring it to your appointment.  This information will help us get to know you and, will expedite the process of "history gathering" during your sessions.

The form is 4 pages in length.  When you arrive on the Form Page, scroll down to the bottom of the screen and click on the PRINT button, located in the bottom, right-hand corner of the page.

The Form will print in Black and White.
